Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends.


Compensation: Starting annual salary is $49,350.86. Being bilingual is not a requirement for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin. What you'll be Responsible for:

Essential functions of the position are below. Additional duties may be assigned as required.

In addition to maintaining your own caseload, being compliant with you monthly work, and attending weekly team meetings mentors will be responsible for the following:

  • Involved in orientation & training of new staff
  • New staff will shadow mentor to learn how to prepare for an assessment, conduct an assessment, and complete necessary paperwork
  • Mentor will shadow new hire and provides feedback
  • On-going shadowing of new hire if needed for redeterminations and reassessments
  • Attending weekly mentor meeting
  • Responsible for answering New CM Questions Distribution List
  • Create training materials for mentor trainings

Qualifications:

  • A Bachelor's degree in Social Work or a related field.
  • Social Work license preferred.
  • Knowledge of and experience working with older adults in a community-based setting preferred.
  • Strong interviewing and assessment skills.
  • Strong interpersonal and organizational skills.
  • Excellent written and verbal communication skills.
  • Computer skills: Proficient with MS Office Suite and entering narrative and other data into a database. Ability to use multiple databases. Ability to use the Internet to conduct information searches
  • Private transportation.
