Complex Care Team Manager

The Complex Team Care Manager is responsible for building partnerships and leading initiatives to ensure our agency is able to meet the behavioral health needs of consumers in the community.

Schedule: This is a full-time, non-exempt (35 hour/week) position. We offer a 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends.

Job Activities

Job activities of the position are below. Additional duties may be assigned as required.

  1. Lead in supervising, mentoring, and training complex care staff
  2. Outreach to health professionals in community to develop partnerships and referral relationships
  3. Develop, maintain and implement policies and procedures related to complex care education and programs
  4. Assure compliance with state and grant funded reporting requirements
  5. Educate and consult with agency staff and community members on available resources
  6. Host education events at supportive housing, community centers, senior centers
  7. Actively participate and represent MVES in community meetings, committees and outreach events

Qualifications:

  • A Bachelor's degree with a focus on Behavioral Health, Nursing, Public HHHealth or a related field
  • 5-7 years of behavioral health experience in a community-based organization preferably
  • 2-5 years of management/supervisory experience in a human service/community-based setting
  • Strong analytical, managerial, communication, and leadership skills
  • Project management skills
  • The ability to manage multiple high priority objectives/projects
  • In-depth understanding of MA behavioral health industry
  • Excellent interpersonal skills with demonstrated ability to communicate across all levels, including staff, board members and external partners
  • Highly resourceful team player, with the ability to work effectively independently or as a team
  • Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved
  • Expertise in the following platforms: Microsoft Office (Word, Outlook, Teams, Excel, and PowerPoint), Adobe Pro, Zoom and other web-based applications
  • Experience working in a 501c3 organization and managing in a union environment strongly preferred

Physical Requirements of the Job:

  • Ability to lift, push, pull and carry objects weighing up to 25 pounds
  • Ability to climb up and down stairs unassisted
  • Ability to travel to the community to perform visits at clients' homes
  • Work requires regular standing, stooping, and bending
