Act as the onsite point of contact for residents, families, and skilled nursing facility (SNF) staff as relates to transitions from SNF to the community; support and ensure the most effective transitions for residents including home visits post-hospitalization. Key activities of the position are below. Additional duties may be assigned as required.


Schedule: This is a full-time, non-exempt (35 hour/week) position. We offer 4-day work week and hybrid work schedule (once training is complete.) No nights or weekends.

What you'll be Responsible for:

Essential functions of the position are below. Additional duties may be assigned as required.

  • Have an onsite presence at each assigned nursing facility on a weekly basis.
  • Identify all new nursing facility residents within the nursing facilities in the MVES catchment and visit with residents to increase awareness of community supports and services and introduce transition as a potential option.
  • Assist residents who expresses interest in transitioning to the community with determining most appropriate supportive program and conduct assessments to determine eligibility.
  • Begin the process of gathering all necessary documentation and identification needed for application for housing and other public benefits.
  • Complete referrals to other programs and follow-up on referrals to ensure timely transition.

Qualifications:

  • A Bachelors' degree required from an accredited school in Social Work, Psychology, Counseling, Human Development, Nursing, Sociology, Criminal Justice, Public Health, Public Policy, or Gerontology plus at least three years of experience in counseling, casework or case management providing protective or crisis intervention services.
  • The ability to manage multiple objectives/projects
  • Excellent interpersonal skills with demonstrated ability to communicate across all levels,
  • Highly resourceful team player, with the ability to work effectively independently or as a team
  • Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved
  • Expertise in the following platforms: Microsoft Office (Word, Outlook, Teams, Excel, and PowerPoint), Adobe Pro, Zoom and other web-based applications

Physical Requirements of the Job:

  • Ability to lift, push, pull and carry objects weighing up to 25 pounds
  • Ability to climb up and down stairs unassisted
  • Ability to travel to the community to perform visits at clients' homes
  • Work requires regular standing, stooping, and bending
