This is a full-time, exempt (35 hour/week) position. We offer a hybrid remote schedule (1 day/week in the office) and the option of a flexible 4-day work week. Our office is located in Malden, MA.

What you'll be Responsible for:

Essential functions of the position are below. Additional duties may be assigned as required.

  • Coordinate and oversee the completion of the Comprehensive Assessments (CA) for referred enrollees.
  • Review/sign off on Comprehensive Assessments as indicated.
  • Coordinate the development, implementation, and ongoing review of the Person-Centered Treatment Plan (PCTP) inclusive of any LTSS and / or SDH needs or goals of the Enrollee.
  • Review/sign off on Person-Centered Treatment Plan of Care Team enrollees.
  • Conduct initial and ongoing risk assessment; design personal crisis management plans, relapse prevention and harm reduction strategies with members who have been identified as behaviorally complex.
  • Organize and facilitate the effective functioning of the Interdisciplinary Care Team (ICT), including coordinating meetings, facilitating communication, and documentation.
  • Oversee submission of CA, PCTP and all PCTP updates in accordance with the data sharing agreement CP and ACO/MCO Plan.
  • Work with the Integrated Care Programs Manager to oversee compliance and performance in regards to model of care standards and contractual agreements. Address issues related to quality, outcomes, team, and ICC performance as needed.
  • Collaborate closely with PCP and other providers, including but not limited to, community resources, to assure appropriate referrals based on level of care needed to optimize outcomes and minimize risk.
  • Collaborate with ACO Plan, PCP, and other providers regarding changes in services, care transitions, crisis intervention while focusing on continuity and quality of client care and potential efficiencies and cost-savings.
  • Assist with care transitions through collaboration with Enrollee, community provider staff, ICT and hospital staff to ensure a safe discharge plan and a well-coordinated implementation of that plan.
  • Conduct visits and assessments as required in various settings including enrollee's homes, community settings, and/or medical facilities as needed.
  • Monitor case records for accuracy, completeness, timeliness, and continuity.
  • Ensure that quality assurance and customer service standards are met.
  • Encourage staff development through supervision and training.
  • Evaluate individual staff performance and hold staff accountable for meeting standards.
  • Participate in the interviewing and selection process for hiring new staff.
  • Participate in the orientation and training process for new staff.
  • Compile and submit required statistics and reports as needed.
  • Perform other duties, as required.

Qualifications:

  • Master's Degree required; License (LPHA) to practice as "Licensed Practitioner of the Healing Arts" in MA required (LICSW, LCSW, LMHC, LADC I, or RN with background in behavioral health)
  • MA license to practice independently (LICSW, LMHC) preferred.
  • Minimum of 5 years clinical and case management experienced preferred.
  • Supervisory experience preferred.
  • Preference given to and those with lived experience of psychiatric conditions.
  • Ability to effectively represent the organization in a variety of circumstances and forums.
  • Knowledge of person-centered, strengths-based, recovery-oriented values and principles and modalities. Knowledge of motivational interviewing, stage of change, harm reduction, clinical and psychiatric rehabilitation values, principles, and techniques.
  • Knowledge of formal and informal assessment practices
  • Ability to build strong relationships. Skills and competence to establish supportive trusting relationships with Enrollees and caregivers.
  • Knowledge of MassHealth, state agencies, provider service system, and community-based resources available to serve persons with disabilities.
  • Cultural competency and the ability to provide informed advocacy for a diverse population of consumers. Knowledge of trauma-informed care. Sensitivity to the cultural, religious, ethnic, disability, and gender issues
  • Ability to effectively facilitate meetings, support, mentor, and supervise staff.
  • Ability to work in a professional and confidential capacity, knowledge of HIPAA requirements.
  • Ability to prioritize, problem solve, transition among tasks, and take initiative as appropriate.
  • Ability to make sound decisions and demonstrate strong ethics and professional boundaries.
  • Computer skills: Proficient with MS Office Suite, and entering narrative and other data into a database. Working knowledge of Excel. Ability to use the Internet to conduct information searches.

Driving Requirements:

  • Driving is a requirement for this position using a personal vehicle. You must possess and maintain adequate insurance as well as maintain a safe driving record which is subject to annual checks. A valid driver's license must be presented at the time of employment. Incumbents must be at least 21 years of age, have maintained a valid US driver's license for at least one year, and must be able to pass a driver's screening background check.
