Complex Care Caseworker
The Complex Care Caseworker will access the needs of individuals experiencing complex care needs and provide resources/referrals with a focus on enrollment into the ANCHOR Program.
Schedule: This is a Local Union 509, full-time, non-exempt (35 hour/week) position. We offer 4-day work week (once training is complete.) No nights or weekends.
Compensation: Starting annual salary is $52,722.12. Being bilingual is preferred for this role, but we recognize it as an asset and offer an additional 6% bilingual designation for Spanish, Haitian Creole, Russian, Vietnamese, Cantonese or Mandarin. What you'll be responsible for:
The key activities of the position are below. Additional duties may be assigned as required.
Top Key Activities:
- Serve as the ANCHOR Program case manager and maintain compliance with ANCHOR program, grant, state and agency program requirements
- Process referrals from internal and external services/programs, while meeting timeline and documentation standards
- Actively participate in interdisciplinary team meetings
- Act as a resource to MVES staff dealing with individuals with complex care issues
- Provide support to consumers facing complex care needs
Other Activities:
- Cross collaboration with all departments for best programs to utilize
- Offer training opportunities to internal staff
- Outreach and presentations to community on Complex Care
Qualifications:
- A Bachelors' degree from an accredited school in Behavioral Health, Psychology, Counseling, Criminal Justice, Public Health, or Gerontology plus at least three years of experience in behavioral health or casework or case management providing protective or crisis intervention services
- Previous experience in a human service/community-based setting preferred
- Bilingual preferred
- The ability to manage multiple objectives
- Excellent interpersonal skills with demonstrated ability to communicate across all levels
- Highly resourceful team player, with the ability to work effectively independently or as a team
- Demonstrates a willingness to take on new tasks with a general attitude that no task is too small, impossible, or cannot be improved
